BEFORE YOUR MEDICAL SPA VISIT

SCHEDULING APPOINTMENTS
It is recommended that you call in advance to schedule your appointment. Our receptionists will be happy to help you schedule a time most convenient for you. A credit card number, or a gift card certificate number, must be provided for all reservations.

FIRST VISIT
Please arrive 15 minutes prior to your first treatment to allow ample time to check-in, fill out appropriate paperwork and prepare for your treatment.

MEDICAL QUESTIONNAIRE
All clients and patients of AVIVA MedSpa & Cosmetic Surgery Center are required to fill out a medical health questionnaire. You may download our Client Questionnaire and fill it out in the convenience of your own home.

CANCELLATION POLICY
Due to staffing requirements, and as a courtesy to our other clients, please notify us of AVIVA MedSpa cancellations at least 24 hours in advance, and 48 hours in advance for physician appointments. If a cancellation is not made, a $50 fee will be charged. Our cancellation policy is strictly enforced, with no exceptions.

GIFT CERTIFICATES
Gift Certificates and Gift Cards are available for any service, package or dollar amount.

CELL PHONES
For the comfort of all clients, please turn off or silence your cell phone while at the spa. Also, please keep conversations at a quiet level.

SMOKING
We are a smoke-free environment. Smoking is not permitted anywhere within our facility.

PRICES
All prices are subject to change without notification.

PAYMENT OF SERVICES
We request payment for consultations and treatments at the time the service is provided. We accept Visa, MasterCard, American Express, Discover Card personal checks, cash and CareCredit. There will be a $25 charge on all returned checks. You may download our Financial Policies.

CARECREDIT
CareCredit is a credit card offered exclusively to medical providers. A product of GE Money, the card offers convenient payment options, giving you the means to spread out the payments for your procedure. Payment plans are available for every financing need, including interest-free for twelve to eighteen months, with low monthly payments and no upfront costs. GRATUITIES
Gratuities are not accepted or necessary, and they are not included in the cost of treatments and services, including facials and massages. We pay our highly experienced staff so you do not need to.

DRESS CODE
Relax! Your visit is a day for you. Dress any way you find most comfortable. For cosmetic surgery procedures, special instructions will be provided.

CONSULTATIONS
Consultations with the medical staff are complimentary and include a VISIA Skin Analysis. On average, the consultation takes 30 minutes. Please see the Cosmetic Surgery section of this website for more information about cosmetic surgery consultations.

PARKING
We are conveniently located in a new medical office building with ample parking.

RETURN POLICY
It is not always possible to predict how the skin will respond to skin care products. Should a reaction occur, it would be necessary for you to consult with your AVIVA MedSpa skin care specialist prior to exchanging or returning your product. Exchanges or returns must be made within 30 days of purchase and are for credit only. We do not offer refunds on services, however exchanges for other like services will be considered.

CHILDREN
In order to achieve the high level of service that we promise our guests, we request that all guests be a minimum of 12 years of age and any guest under the age of 18 be accompanied by a parent. We appreciate your understanding. We cannot be responsible for children while you are receiving a treatment. Additionally, children will not be allowed in treatment rooms.

PETS
To ensure safety, cleanliness and the comfort of all clients, pets are not permitted in the spa.

COURTESY
We at AVIVA MedSpa & Cosmetic Surgery Center adhere to all HIPAA confidentiality requirements. We ask that you also respect the confidentiality of other guests in the spa. You may download our Privacy Practices.

PUNCTUALITY
We’re all late from time to time. So, if you are going to be late, please be courteous and give us a call. We will do our best to adjust or reschedule your treatment time. However, we cannot infringe on the appointments of clients who arrive promptly. We will continue to see these client at their appointed times.

PERSONAL ITEMS
Please leave valuable items and jewelry at home. AVIVA MedSpa & Cosmetic Surgery Center employees are not responsible for lost, stolen, or damaged items.

WHEEL CHAIR ACCESS
Need a lift? We can pick you up at the door with a wheelchair.

RIGHT TO INFORMATION
You have the right to inspect and obtain a copy of the health information that may be used to make decisions about you, including patient medical records and billing records. You must submit your request in writing to:
